Thank you for choosing Pediatrics at Oyster Point as your child’s medical home, where we strive to offer a comfortable, caring environment so our patients and their families can feel at ease. We’ve included helpful items on this page to ensure you and your child’s first trip to our office is productive, stress-free and enjoyable.

Coming to Your Appointment

If this is your first visit to our office, completed new patient forms will be required. You can download them, as well as review our policies, on the Forms and Policies page of our website and bring them with you or fill them out when you arrive in our office. You will need to present your insurance card, photo ID and copay, if required by your insurance company. Parents are encouraged to arrive 15 minutes prior to their appointment time to allow adequate time for all steps to be completed.

What to Expect

We review growth and development, perform a physical examination, and recommend any other anticipatory actions, testing, and immunizations that are indicated for your child’s specific age recommendations.

Documents you need to Bring

  • Child’s full medical history
  • Vaccination Records
  • Insurance card(s)
  • List of any medications and dosages your child is currently taking
  • List of questions and refill requests
  • Your driver’s license, or valid photo ID
  • Family health history
  • School Forms and/or Sports Physical Forms, if needed
